The incident form is intended for you to formally report and incident or something serious that you have experienced or witnessed happen involving the club.
This information will be passed onto the president, who will present a summary of all completed PET forms to the committee at our regular meetings and the information will be used to make improvements to our club and/or be reported to the appropriate bodies.
